Listening to him and Billie talk about things in the Doomsday in vision clip, it seems he is largely interpreting the character himself. You also see this in the confidentials. I'm sure that they tell him things like...cry, hop about and give us a bit of giddiness, though. And the way he handles Martha...was no doubt definitely discussed with Russell. I imagine a lot of it gets worked out at the run through with everyone offering suggestions and the writer on hand to point out his/her vision of the moments.
Still...it is easier to tell David to give you a distracted scratch of the head and a pout than it is to describe it. Such is the cross I, and my fellow Ten/Rose writers, bear. ;-D
